Always There Family Home Health Services is the right choice for any patient who needs skilled, in–home nursing
care and whose insurance requires that the care is provided by a certified agency.
All care is provided by licensed home health professionals, including registered professional nurses, physical and
occupational therapists, speech language pathologists, dietitians and medical social workers.
Nursing Services —
Skilled nursing services provided by a Registered Nurse or a Licensed Practical Nurse,
under the supervision of an RN.

Medical Social Work
- Assessment of the social and emotional factors related to the patient’s illness, need for care, response to treatment and adjustment to care.
- Assessment of the relationship of the patient’s medical and nursing requirements to the patient’s
home situation, financial resources and availability of community resources.
- Appropriate action to obtain available community resources to assist in resolving the patient’s problem.
Mental Health Program —

Intravenous Therapy —
Services provided by RN’s
- Administration of IV medications
- Maintenance of IV access lines such as PICC lines and ports
- Teaching caregivers to administer IV medications
Telehealth
- Monitor blood pressure, pulse, weight and blood glucose at home. Information is relayed to secure website via the
telephone.
- Patients also report their subjective symptoms and medication compliance. Nurses monitor data daily and contact
patient and/or primary care physician when abnormal data is identified, in order to adjust careplan to prevent medical crisis or hospitalization.
- Medication management box dispenses medications and tracks missed doses. Information is relayed to secure website via the telephone. Nurses monitor website and follow up with the patient when missed doses occur. Thus, improving health through better medication management.
Wound V.A.C.®Therapy
